Foxos digital continuity of care model is rapidly transforming patient access across Australia, with growing adoption across public and private healthcare settings. Deployed across many institutions including as Gold Coast University Hospital (GCUH) and Sydney Adventist Hospital (the San), this innovative approach is streamlining care, reducing wait times, and enhancing engagement for patients with chronic conditions such as rheumatoid arthritis, diabetes, and asthma, and in making bookings and appointments with health services.

Traditionally referred to as a digital front door to care, Foxo has reimagined this concept into a digital door to care that stays open, enabling continuous, seamless engagement with hospital healthcare services. Thousands of patients are now connecting directly with administrative teams and healthcare professionals to find healthcare services and book appointments across various departments. This shift is easing the burden on critical resources and significantly reducing reliance on traditional phone queues.

Foxos Patient Connect module, built on Microsoft Azure, also allows patients to securely message, share files, or request information, as well as have video or voice calls with healthcare staff, all through a simple, no-app-required mobile interface. This approach eliminates app fatigue and improves equitable access to digital healthcare, ensuring all patients can engage as guests in a secure, user-friendly environment.

The new digital patient pathway has significantly enhanced patient engagement, especially for those with chronic conditions who need ongoing support. Weve seen strong uptake, with patients valuing the ease of access and continuity of care. This model is reducing unnecessary hospital visits while ensuring timely and effective patient management. Dr Jenni Ng, Medical Director, Rheumatology, Gold Coast Health.

In Sydney Adventist Hospitals medical imaging services, the Foxo Patient Connect module had an immediate take-up with over 1,200 patient cases within the first 12 weeks of the program, with a conversion rate of 87% from digital inquiry to scan attendance. These bookings and inquiries transact directly between the patient and the SAN Imaging staff in their Microsoft Teams account via the Foxo MS Teams app.
